Abstract [sv]

De vanligaste orsakerna till sjukskrivning i Sverige och många andra länder är psykisk och muskuloskeletal ohälsa. Syftet med denna översikt av översikter är att bedöma det vetenskapliga stödet för vilka faktorer som bidrar till att personer som är sjukskrivna i dessa diagnoser kan återgå i arbete. Översikten baseras på tidigare gjorda vetenskapligt granskade litteraturöversikter kring faktorer som bidrar till återgång i arbete (Return To Work, RTW). Utifrån rådande kunskapsläge har översikten inriktats på interventioner där åtgärder på arbetsplatsen ingår. Översikten ska öka kunskapen om vilka psykologiska och sociala komponenter på arbetsplatsen som bidrar till förbättrad och hållbar RTW.

Forskningsfrågan kan också uttryckas som: Vilka förändringar och insatser är verksamma för vilka sjukskrivna i vilka sammanhang? Ett ytterligare mål är att översikten ska bidra till en syntes inom RTW-området för att bland annat identifiera kunskapsluckor. Översikter söktes i databaserna PubMed/Medline, Scopus, Psychinfo, CINAHL, Cochrane Library och Sociological Abstracts för perioden januari 2018 till april 2024. Totalt identifierades 203 artiklar som granskades av två oberoende forskare utifrån relevansoch kvalitet.

Bedömningarna genomfördes enligt principerna i verktyget AMSTAR. 30 översiktsartiklar bedömdes som relevanta och av tillräckligt hög kvalitet för att inkluderas. Elva översikter avsåg psykisk ohälsa, åtta muskuloskeletal ohälsa och elva båda områdena. Dessa översikter bygger på totalt cirka 1500 originalartiklar, varav majoriteten är genomförda i Sverige, Norge, Finland, Danmark och Nederländerna.

Översiktens analys av faktorer som sammanhänger med RTW tar främst upp följande områden och aspekter: multimodalitet, det vill säga teamarbete av behandlare från olika professioner, koordinering av behandling och arbetsplatsinterventioner, intressenter, individcentrerade och skräddarsydda arbetsplatsinterventioner, stöd och sociala kontakter, klassiska arbetsmiljökomponenter som krav, kontroll och stöd samt personrelaterade faktorer som betydelsen av förväntningar och tro på egna förmågan.

En översikt jämförde RTW bland kvinnor och män. Översikterna demonstrerar återgångsprocessens komplexitet, men trots komplexiteten framkommer tydliga mönster. Praktiskt taget alla översikter visar att multimodala behandlingsinsatser, som kombineras och koordineras med olika former av arbetsplatsmodifieringar och där den sjukskrivne själv aktivt deltar, är effektiva för RTW. Arbetsplatsinsatserna omfattar arbetsanpassning, förändrad arbetsbörda, förändrade arbetsuppgifter, ökad kontroll och inflytande i arbetet, problemlösningssamtal, socialt stöd från arbetsledare och arbetskamrater. Exempel på multimodala insatser vid psykisk ohälsa är medicinering, psykoterapi (KBT, IPS, ACT) och stresshantering/avslappningsträning och vid muskuloskeletal ohälsa smärtlindring, sjukgymnastik, arbetsterapi och fysisk träning. Översikten kring RTW bland kvinnor och män visar att traditionella könsrollsmönster på olika sätt påverkar kvinnors och mäns möjligheter till RTW.

Tio översikter som omfattar fler än 150 randomiserade kontrollerade originalstudier, där aktiva interventioner på arbetsplatsen för RTW utvärderats, ger extra tyngd åt resultat och slutsatser, särskilt då dessa översikter är mycket samstämmiga.

I översiktens syntesavsnitt diskuteras kunskapsluckor och forskningsbehov vad gäller arbetsplatsmodifieringar, koordinering av återgångsprocessen, personrelaterade faktorer och individcentrering, byte till annan arbetsgivare, köns- och genusfrågor med flera aspekter. Heterogeniteten i utfallsmått och uppföljningstider är stor, vilket försvårar områdets kunskapstillväxt, bland annat vad gäller den viktiga frågan om långsiktig hållbarhet i olika insatser.

Sammanfattningsvis är skräddarsydda interventioner nyckelfaktorer för framgångsrik RTW. Dessa består av en kombination av insatser och arbetsplatsförändringar anpassade till den enskilde individen. Det gäller såväl för sjukskrivna i psykisk som i muskuloskeletal ohälsa.

I beskrivningar av arbetslivet talas det ofta om det nya men inte sällan är det nygamla frågor som uppenbarar sig i en ny kontext. Det finns en kontinuitet till tidigare förhållanden som är värd att uppmärksamma inte minst i fråga om makt, sociala relationer och mänskliga behov. Digitala infrastrukturer genomsyrar nu arbetslivet och dessa kan ses som en ny produktionsform i relation till tidigare agrara och industriella former. I kapitlet undersöks och jämförs anställningsvillkor och anställningssystem i dessa tre produktionsformer. Gemensamt är grundkonflikten mellan den enskilda individens behov av kontroll, trygghet, mening och förutsägbarheti livet och företags och organisationers intresse av snabbar anpassningar till omvärldsförändringar. Konflikten formar konstruktionenav anställnings- och anknytningsformer både i kvalitativ och i kvantitativ mening. Kapitlet har i grunden ett psykologiskt perspektiv med fokus på arbetsvillkor och hälsa i två grupper av anställda -inlåsta och utlåsta - med stor diskrepans mellan sin aktuella och sin önskvärda arbets- och livssitutation

Note

Descriptions of working life often talk about the new, but not infrequently it is new old questions that appear in a new context. There is a continuity to previous relationships that is worth noting, not least in terms of power, social relations and human needs. Digital infrastructures now permeate working life and these can be seen as a new form of production in relation to previous agrarian and industrial forms. The chapter examines and compares employment conditions and employment systems in these three forms of production. Common is the basic conflict between the individual's need for control, security, meaning and predictability in life and companies' and organizations' interest in rapid adaptation to changes in the world around them. The conflict shapes the construction of employment and attachment forms both in a qualitative and quantitative sense. The chapter basically has a psychological perspective with a focus on working conditions and health in two groups of employees – locked in and locked out – with a large discrepancy between their current and their desired work and life situation.

Under covid-19-pandemins första våg, tidigt våren 2020, valde de flesta länder, inklusive Sverige, att rekommendera eller kräva att de anställda som kunde skulle arbeta på distans från bostaden för att minska smittspridningen på arbetsplatser och under resor till och från dessa arbetsplatser. Att arbeta på distans skiljer sig i många avseenden från att arbeta i arbetsgivarens lokaler.

Det övergripande syftet med denna översikt, som består av tre delar, har varit att sammanställa och öka den forskningsbaserade kunskapen om distansarbete i hemmet.

Översikten omfattar forskningslitteratur inom tre områden:

  • arbetsmiljö och hälsa
  • balans mellan arbetsliv och privatliv
  • produktivitet.

I analyserna har vi därtill särskilt fokuserat på likheter och skillnader i mäns och kvinnors villkor vid lönearbete i hemmet. Ytterligare syften har varit att undersöka och dra slutsatser om distansarbete i hemmet under tiden före respektive under pandemin.

Abstract [en]

During the first wave of the COVID-19 pandemic in early spring 2020, most countries, including Sweden, chose to recommend or require employees for whom doing so was possible to work remotely from home in order to reduce the spread of infection at workplaces, and during commutes to and from workplaces. Working remotely differs in many respects from working at theemployer’s premises. The overall objective of this three-part review has been to compile and increase research-based knowledge of remote work from home.

This review encompasses research literature in three areas:

  • work environment and health
  • work–life balance
  • productivity.

In the analyses, we have therefore focused in particular on similarities and differences in the conditions for men and women regarding paid work at home. Additional aims were to investigate and draw conclusions about remote work from home during the period before and during the pandemic, respectively.

The first part is a review of research reviews of remote work before the pandemic (published 2005–2021), while the second part is a review of original studies of remote work from home during the pandemic (2020– 2021). Part three comments on and summarises the material presented in parts one and two. The main purpose of the division into before and after is that remote work before the pandemic was primarily voluntary and planned, while remote work from home during the pandemic was unexpected, with minimal planning, and more or less forced.

The review has been limited to remote work from home conducted by employees through an internet connection.

Abstract [en]

Objective: To analyze the associations between favorable physical and psychosocial work factors and health behavior among healthcare employees (nurses and care assistants) with health complaints.

Methods: The study was based on seven iterations (2001-2013) of a biennial Swedish work environment survey linked with data from public registers. In all, 7,180 healthcare employees, aged 16-64 years, who had reported health complaints, were included. Health behavior was operationalized through four combinations of sickness absence (SA) and sickness presence (SP): 'good health behavior' (Low SP/Low SA), 'recovery behavior' (Low SP/High SA), 'risk behavior' (High SP/Low SA), and 'poor health behavior' (High SP/High SA). Odds ratios (OR) were calculated by multinomial logistic regression with 95% confidence intervals (CI).

Results: After adjusting for socio-demographic factors, those who rarely worked in strenuous postures had an increased probability of having 'good health behavior' (OR range: nurses 1.72-2.02; care assistants 1.46-1.75). Those who rarely experienced high job demands had increased odds for having 'good health behavior' (OR: nurses 1.81; OR range: care assistants 1.67-2.13), while having good job control was found to be related to 'good health behavior' only among care assistants (OR range 1.30-1.68). In the full model, after also considering differences in health, none of the work environment indicators affected 'good health behavior' among nursing professionals. Among care assistants, rarely having heavy physical work and having low psychosocial demands remained significantly associated with 'good health behavior' (OR range: 1.24-1.58) and 'recovery behavior' (OR range: 1.33-1.70). No associations were found between favorable work environment factors and 'risk behavior' among the two groups of employees. However, positive assessments of the work situation were associated with 'good health behavior,' even after controlling for all confounders for both groups (OR range: 1.43-2.69).

Conclusions: 'Good health behavior' and 'recovery behavior' among care assistants were associated with favorable physical and psychosocial working conditions even when health was considered. This implies that reduced sickness presence and sickness absence among care assistants can be achieved through improved physical and psychosocial working conditions.

Abstract [en]

Purpose: The first objective was to contribute to a better understanding of the contrasting and paradoxical results in studies of work environment factors and sickness presence and sickness absence. A second objective was to examine if, and under what conditions, employees choose to replace sickness absence with sickness presence, i.e., so-called substitution.

Methods: The study utilizes a large body of cross-sectional questionnaire data (n = 130,161) gathered in Sweden from 2002 to 2007 in connection with a comprehensive health promotion initiative. Health and motivation were analyzed as mediators of the effects of five job factors, job control, job support, job demand, role conflict and “work to family conflict” on sickness presence and absence.

Results: The results concerning job demands indicate substitution in that increased job demands are associated with increased presenteeism and reduced absenteeism. The direct effect of higher job support was increased absenteeism, but via the health and motivation paths, the total effect of more social support was health-promoting and associated with a reduction in sickness absence and sickness presence. High job control emerged as the most pronounced health-promoting factor, reducing sickness presenteeism as well as absenteeism. More role conflicts and work-to-family conflicts were directly and indirectly associated with decreased health and increased absenteeism as well as presenteeism. earlier research.

Conclusion: The mediation analyzes shed light on some of the paradoxes in research on sickness presenteeism and sickness absenteeism, especially regarding job demands and job support. The substitution effect is important for workplace policy and occupational health practice.

Abstract [en]

Objective: To describe if health complaints relate to health behavior in terms of sickness absence (SA) and sickness presence (SP) and to examine how complaints and health behavior predicts the risk for future long-term sickness absence (LTSA).

Methods: Data originates from work environment surveys 2001 to 2013 and SA registers 2002 to 2016 of 1838 nurses, 7430 care assistants, and 40,515 individuals in all other occupations. Descriptive and regression analyses were conducted.

Results: Physical complaints and high SA in combination with high SP increased the risk of LTSA among nurses and care assistants. Nurses' high SP and care assistants' high SA elevated the LTSA risk.

Conclusions: Strategies to reduce the reasons behind physical health complaints among health care workers are warranted. SP among nurses and SA among care assistants should be considered in the organization of their job demands.

Abstract [en]

This study examined exposure changes in three psychosocial dimensions - job demands, job control, and social support - and the associations between these dimensions and sickness absence throughout the period 1991-2013. The analyses covered periods of economic ups and downs in Sweden and periods involving major fluctuations in sickness absence. Data on care workers (n = 16,179) and a comparison group of employees in other occupations (n = 82,070) were derived from the biennial Swedish Work Environment Survey and linked to register data on sickness absence. Eight exposure profiles, based on combinations of demands, control, and support, were formed. The proportion of individuals with work profiles involving high demands doubled among care workers (14%-29%) while increasing modestly in the comparison group (17%-21%) 1991-2013. The work profile that isolated high-strain (iso-strain), i.e., high demands, low control, and low social support, was more prevalent among care workers, from 4% in 1991 to 11% in 2013. Individuals with work profiles involving highdemand jobs had the highest number of days on sickness absence during the study period and those with the isostrain work profile had the highest increase in sickness absence, from 15 days per year during 1993-1994, to 42 days during 2000-2002. Employees with a passive work profile (low job demands and low job control) had the lowest rate and the lowest increase in sickness absence. Individuals with active work profiles, where high demands are supposed to be balanced by high job control, had a rather high increase in sickness days around 2000. A conclusion is that there is a long-term trend towards jobs with high demands. This trend is stronger among care workers than among other occupations. These levels of job demands seem to be at such a level that it is difficult to compensate for with higher job control and social support.

Kapittelet tar utgangspunkt i et forskningsarbeid som omhandler tillit i veiledning. Informantene er lærere i veilederutdanning som har dokumentert sine opplevelser, oppfatninger og førstehåndserfaringer med tillit. På bakgrunn av lærernes gjennomførte veiledninger med «kjente» og «ukjente» veisøkere diskuteres tillit i veiledning i lys av tre områder som informantene har trukket fram som betydningsfulle ut fra erfarte oppdagelser. Det er interpersonlig tillit, trygghet og makt.

The objective of this study was to investigate if the psychosocial work environment moderates the proposed negative impact of presenteeism on future general health. We expect that the negative impact of presenteeism on general health is weaker if the psychosocial work environment is resourceful, and more pronounced if the environment is stressful. Data were derived from the 2008-2018 biennial waves of the Swedish Longitudinal Occupational Survey of Health (SLOSH). The final analytic sample consisted ofn= 15,779 individuals. We applied repeated measures regression analyses through generalized estimating equations (GEE). Results from the autoregressive GEE models showed statistically significant interaction terms between presenteeism and all four investigated moderators, i.e., job demands, job control, job support and job strain. The results indicate that the psychosocial work environment moderates the negative association between presenteeism and general health and illustrates a buffering effect of the psychosocial work environment. A possible explanation for these results may be that psychosocially resourceful work environments give room for adjustments in the work situation and facilitate recovery. The results also indicate that by investing the psychosocial work environment employers may be able to promote worker health as well as prevent reduced job performance due to presenteeism.
