    Abstract [en]

    Mobility in and out of intimate relationships has become more common in late modern societies also in later life. However, it has been a neglected issue in social gerontology and sociological studies on ageing. In this paper the research questions are: What characterizes the formation of new inti-mate relationships in later life? Are there any specific, more or less univer-sal, conditions that separate them from relationships in earlier life phases? Qualitative interviews was used with a strategical sampel,consisting of 28 Swedes, 63–91 years, who have established a new intimate heterosexual relationship after the age of 60 or who are dating. The results showed Time constituting a central structuring condition for new intimate relsionships in later life. In the results three aspects of time – Available free time, Lived time and Remaining time – which all have a constituting and an important formative power on new late in life relationships are discussed in relation to theories of late modernity and the Third Age and in relation to changing demographical conditions

    The purpose of this study was to examine how some social workers in Sweden describe the social work with unaccompanied children. Emphasis was on the role of culture in social work with the unaccompanied children. I wanted to examine how the social workers describe their view on social work with unaccompanied. I also wanted to examine how the social workers describe their view at their own and the unaccompanied children’s cultural backgrounds influence on the social work with the unaccompanied. The study is based on qualitative interviews with four social workers. The empirical material was analysed based on the theories ethnocentrism and symbolic interactionism. The results of the study shows that both the social workers and the unaccompanied children’s cultural background effects social work. The social workers cultural background has a particular effect on the meetings with the unaccompanied children. The social workers capacity to fully understand the unaccompanied clients is limited for cultural factors.

    Abstract [en]

    Treatent processes, what is being done and why it is being done in treatment arrangements, is a field of substane abuse study in wich relatively little research has been done. There are several methodological problems. The method used in this study of care providers´perspectives in Swedish LVM-institutions (institutions för the coercive care of substance abusers) i the focus group. In order to get a comprehensive approach, a strategic selection of four institutions was made, based on the institutions´therapeutic or pedagogical viewpoint. The analyses reveal that it is impossible to discern an explicit description of what constitutesmotivational work at any of the LVM institutions, that the proveders at the four LVM institutions, that the providers at the four LVM institutions gave completely different pictures of coersive care and that they used different tools to accomplish ther central task - to motivate the clients. At three of the institutions the clients´abuse problems were only mentioned in passing in their treatment, and the staff spoke of the clients´resistance and negative attitudes against the coercion as obstacles the had to negotiate in order to continue the motivation work. The most radical strategy was to work as though the coercion did not exist.

    En effekt av rörelsen för evidensbaserad praktik, EBP-rörelsen, är att intresset för det praktiska arbetet som pågår varit svalt, istället har förhoppningarna ställts mot en framtid där klienterna ska erbjudas utvärderingsinstrument och metoder som har vetenskapligt baserad effekt. Kritiska röster menar dock att EBP-rörelsen kan vara en återvändsgränd om den inte tar hänsyn till fler faktorer än bästa evidens. Syftet med denna artikel är att ta det arbete som pågår i praktiken som utgångspunkt. Resultatet visar att socialsekreterarnas erfarenheter av klientarbetet är att deras arbete är komplext och att de inte i förväg kan veta hur det kommer att gå för respektive klient, vilka interventioner som kan vara framgångsrika och vilka olika faktorer som kan komma att ha betydelse för att just denna klient skall påbörja ett förändringsarbete. En implementering av evidensbaserad praktik i denna jordmån bör ta hänsyn till dessa erfarenheter, kunskapsutvecklingen av det sociala arbetet inom socialtjänsten måste också innebära en uppbyggnad av dokunenterad beprövad erfarenhet.

    Abstract [en]

    The importance of family is widespread, both in society in general and within social work

    specifically. This essay focuses on the construction of family in the guidelines concerning

    adoption that the Board of health and welfare issue. The aim of this study is to identify and

    uncover dominant discourses concerning family in the guidelines. To do this we have used a

    form of Critical Discourse Analysis, influenced by Norman Faircloughs three-dimensional

    analytical model. Our major findings were, among others, that different kinds of family

    practices are excluded from the idea of what a family is. This process of exclusion is done

    through two different sets of discourses; discourses of sameness and discourses of difference.

    Discourses of sameness is concerned with including the adoptive family in the norm of the

    “ideal” family, but without really changing the idea of what a family can be. This leaves the

    traditional idea of family to remain unchallenged in the guidelines. Discourses of differences is

    concerned with the construction of the adoptive family as different than the idea of the “ideal”

    family. The consequences of this is that the construction of the “good” family excludes family

    practises that defy the traditional “ideal” family.

    Abstract [sv]

    Uppsatsens syfte var att öka förståelsen och kunskapen om de barn som bevittnat våld mellan närstående i familjen. Frågeställningarna var: 1) Hur beskrivs barns, mödrars och behandlares röster i litteraturen om barn som bevittnat våld i familjen? 2) Vad har våldet för konsekvenser för barnen enligt mödrar och behandlare? 3) Hur diskuteras lagstiftning, insatser och behandling i litteraturen om barn som bevittnat våld i familjen? En selektiv kunskapsöversikt med en illustrativ intervju med en behandlare genomfördes för att besvara frågeställningarna. Resultatet analyserades ur ett multidimensionellt perspektiv med hjälp av socialkonstruktivistisk teori, anknytningsteori samt systemteori med ett rollperspektiv. Det som framkom i studien var att barn som bevittnat allvarliga våldshändelser i hemmet upplevde ett trauma vilket kunde leda till posttraumatiska stressyndrom hos barnet. Över hälften av barnen i litteraturen hade också själva blivit misshandlade. Barnen utvecklade olika copingstrategier för att kunna hantera situationen i familjen. Dessa strategier såg olika ut beroende av faktorer som ålder, utvecklingsfas samt personliga egenskaper hos barnet. För att barnet skulle kunna bearbeta de traumatiska upplevelserna ansågs behandlingsinsatser i form av krissamtal eller gruppverksamhet vara lämpliga åtgärder. Fungerande nätverk i form av goda vuxenkontakter och sunda kamratrelationer ansågs verka skyddande och minska riskerna för att långvariga psykiska problem utvecklades hos barnet.

    Abstract [en]

    This study aims to investigate individual and collective resistance strategies used by social service workers to improve their work situation. It also aims to describe what consequenses their acts of resistance has on an individual, collective and structural level. The study include ten qualitative interviews and three participating observations with social service workers within children and youth unit at the social Services in Stockholm County. The result is analysed with Hollander & Einwohners typology of resistance (2004). The study shows that speaking up towards high work load, union organization and to shift responsibility from the individual to the structural are powerfull ways to change the work load. This can lead to being seen as a trouble-maker, a vulnerable position in the workplace, but also to expanded resources like new positions. To take breaks, work slowly, a good atmosphere and overriding internal requirements for documentation are strategies that can bring stress and guilt but also make a great difference and bring understanding and support from the group. Covert resistance can lead to overt and individual resistance can lead to collective. The gist is that a rested social service worker can make a big difference to an experienced high work load.

    This study examines the construction of credibility in a rape conviction from 2013 in Swedishcourts of law by the district court and the higher regional court. The study use discourseanalysis with a focus on identity, and gender and honour as analytical concepts to analyse theconcept of credibility. The results show that the concept of credibility is not constantthroughout the courts and neither are the identities. In this case it may be due to the districtcourt´s judgement in a general credibility and weighed in only the plaintiffs Romanianethnicity and not the defendant’s Iranian ethnicity, while the higher regional court on theother hand judged a more specific credibility, looked at the facts and ruled out ethnicity. Inthe latter court the defendant was found guilty. The study also shows that when the courtsinterpret the parties from a modern context they miss alternative explanations for theirbehaviour regarding values of clannism in a pre-modern context. They missed alternativeways to interpret the parties as a trustworthy victim and a possible offender respectively. Aresult from this study is the suggestion that the most reliable credibility assessment is tointerpret the parties from their own context.

    Abstract [en]

    This essay is based on a qualitative study of Swedish high school students’ discussions of rape. We have a socialconstructive perspective and use theories of gender and power. The purpose of the study is to acknowledge teenagers’ views of rape, sexual relationships and the law. The study consists of four focusgroupinterviews with eighteen year old students. We divided the students into genderequal groups, two girlsgroups and two boysgroups. The groups were asked to define rape, discuss different examples and their perception of the law. The study shows that the teenagers on a principle level can articulate what is and what is not rape. However, when it comes to discussing actual situations they express ambiguous thoughts. On one hand they claim that everything that is not concentual sex is rape. Nonetheless, at the same time, they show a reluctance to define any of our examples as rape. It is evident that their definition of rape involves violence and/or threats. Their idea of rape is based on the swedish word våldtäkt, which implies that violence is a part of the assault. Since our examples are not violent, but taken from every day situations that occur in teenagers’ lives, they will not call them rape. Although the teenagers are aware that a lot of the sexual assaults that occur happen in relationships, they still maintain the picture of a violent attack or a hostile situation. The teenagers view boys and girls differently and see men as the strong violaters and women as the week victims. It becomes apparent that the teenagers do not have all the knowledge about the new law and that they have trouble with the interpretations.

    In discussions and empirical investigations of the implementation of evidence-based interventions there is often a narrow focus on treatment fidelity. Studying a social services agency trying to incorporate Motivational Interviewing (MI), commonly regarded as evidence-based, this paper problematises a one-sided attention to treatment fidelity by showing how non-fidelity can be constructive from an organisational perspective. While treatment fidelity is one important aspect, it is argued that there might be other rationales to take into account when implementing evidence-based interventions in professional practices.

    Evidence-based practice behind the scenes: How evidence in social work is used and produced2016Doktoravhandling, med artikler (Annet vitenskapelig)
    Abstract [en]

    The aim of this dissertation is to examine empirically what Evidence-based practice (EBP) and its standardized procedures become when put into practice in social work. EBP builds on the idea that professional practice should be based on systematic and reliable knowledge of the interventions and instruments used in this work. This implies a standardization of both research and practice that has been highly contested. Inspired by works within science and technology studies (STS), this dissertation analyses the actual content of the standardized procedures and their uses in social work practice.

    The dissertation examines a ‘critical case’, a substance abuse social services agency that has worked extensively for several years at implementing EBP, and consists of four papers focusing on three standardized procedures used by the agency in order to enact EBP: 1) the Addiction severity index (ASI) assessment instrument; 2) the psychosocial intervention Motivational interviewing, and 3) the decision-making model Critical appraisal (CA). Ethnographic methods were employed to study the agency’s concrete uses of the standardized procedures in daily practice. MI was also followed in the research literature as it became established as an ‘evidence-based’ intervention.

    Fundamentally, the development of the standards of EBP can be a messy and paradoxical process. In the stabilization of MI, its differences and ‘fluidity’ have eventually been made to disappear and left a stable ‘evidence-based’ object.

    Findings from the ethnographic studies show that EBP, as enacted in the agency’s daily practice, is a bureaucratic project where the agency’s managers have decided on and control the use of a set of standards. Thus, what constitutes relevant evidence is based not on professional discussion within the agency but is ultimately determined by the managers.

    In practice, the standards introduce new logics that cause tensions within the agency, tensions which the social workers are left to handle. Main conflicts concern how the client work is ordered and contradictory organizational rationales. The three standards are used to varying extent, which can be understood by examining what they seek to standardize and how they are put to work. CA was not used at all, mainly due to its design. Disregarding organizational rationales that are unavoidable within the social services, it could not be adapted to the agency’s work. With ASI and MI the situation was different, mostly because of their organizational adaptability. ASI could be implemented in several phases of the agency’s work flow resulting in adjustments of both the instrument and the work flow. As a ‘fluid intervention’, MI was constrained by, but also adjustable to the organization. It was thus possible for both ASI and MI to transform and be transformed by pre-existing practices, in effect creating new practices.

    A major conclusion is that EBP and its standardized procedures is a more dynamic and multifaceted process than previously acknowledged in social work. Rather than a deterministic one-way path, there are different kinds, degrees, and mutual transformations of standardization processes, which must be appreciated in research and in practical efforts to implement EBP. Given the importance of the organization in professional social work, there is a need to move away from individualistic conceptions of EBP and to consider what evidence use might mean from an organizational perspective.

    Abstract [en]

    The past two decades have witnessed an increased demand for standardized documentation within the social services field. This have resulted in the proliferation, translation and attempt to use various kinds of standardized instruments to document clients’ needs and outcomes of interventions. Reviewing the social work literature on standardized instruments and providing empirical examples, this article identifies a widespread unsystematic use in everyday practice. This seriously undermines the ability to use gathered information to make reliable quantitative analyses of clients’ outcomes. Based on the concept of ‘situated standardization’ the article argues for a new direction in the attempts to standardize documentation within the social services. Rather than developing standardized instruments outside the social services context, as in current efforts, situated standardization points to the need to develop instruments from within this context. This would arguably produce instruments better suited for the preconditions of social services’ work, and therefore easier to use systematically. Thus, rather than a ‘light’ standardization, this can be described as a different kind of standardization situated in the actual work of the social services. An important implication is that standardized instruments need to be developed together with a close empirical examination of the setting in which it is supposed to be used.

    Reconsidering critical appraisal in social work: Choice, care and organization in real-time treatment decisionsManuskript (preprint) (Annet vitenskapelig)
    Abstract [en]

    This paper seeks to provide an empirically grounded discussion of the critical appraisal model of EBP in social work practice. Studying decisions in practice, the paper looks ethnographically at an attempt to implement critical appraisal in social work practice, and problematizes some of the assumptions underlying this idea. Whereas critical appraisal tends to view treatment decisions as clear-cut events that are made by autonomous social workers, participant observation shows that decisions emerge over time and that they are ‘organizational’ rather than emanating from individual social workers. Drawing on Mol’s (2008) notion of the ‘logic of care’ and findings from studies of organizational decision making, a more practice oriented understanding of treatment decision-making is outlined.

    Abstract [en]

    This paper seeks to provide an empirically grounded discussion of the critical appraisal model of Evidence-based practice (EBP) in social work practice. Studying real-time treatment decisions, the paper looks ethnographically at an attempt to implement critical appraisal in everyday social work practice, and problematizes some of the assumptions underlying this idea. Whereas critical appraisal tends to view treatment decisions as clear-cut events emanating from autonomous social workers, participant observation shows that decisions emerge over time and are ‘organizational’ rather than individual. Drawing on the notion of ‘logic of care’ and findings from studies of organizational decision-making, a more practice oriented understanding of treatment decision-making is outlined.

    Abstract [en]

    Introduced in 1983, Motivational Interviewing (MI) is now widely regarded as an efficacious psychosocial intervention in the addiction treatment field. Drawing on research literature, this article asks how this was achieved. Contrary to common conceptions of the dissemination of psychosocial interventions within the addiction treatment field, this analysis takes neither MI's identity nor its efficacy to be inherent qualities, but sees them as being constructed by various actors. This construction work is described as stabilizing processes. Being loosely structured and flexible, it is suggested that MI can be regarded as a fluid intervention. This has presented difficulties for its subsequent stabilization. As MI has been differently operationalized in clinical trials, it was not obvious at first to talk about the efficacy of MI as a single object of concern in systematical reviews and meta-analyses. The article discusses some of the complexities involved in the production and dissemination of efficacious psychosocial interventions. Compared with other cases, MI displays a somewhat different mode of stabilization. It is argued that MI has been stabilized enough to be considered an efficacious intervention while simultaneously incorporating fluidity, which makes it useful in a wide range of clinical settings.

    AIM - This article explores the implementation and use of the Addiction Severity Index in addiction treatment practice, both as a clinical instrument and as a way of facilitating outcome measurement. This is regarded as incorporating laboratory logic into clinical practice characterised by the logic of care. DATA - The data is based on ethnographic fieldwork in a Swedish metropolitan social service agency known for its systematic ASI work. RESULTS - The findings suggest that much effort must be dedicated to co-ordinate activities in the agency in line with the laboratory logic, making sure that the interviews are administered systematically. In use, the ASI and the variables in clinical practice are adjusted to each other, making it possible to follow both logics at the same time. In some cases, however, there is a conflict: the ASI becomes an extra task that does not further the clinical work. Once collected, the ASI data must be co-ordinated in line with other information. This has not yet been realised in the agency, which makes the value of the ASI data unknown. CONCLUSIONS - It requires hard work to handle the two logics simultaneously in addiction treatment practice: activities must be co-ordinated, and instruments and variables in clinical practice must be continuously tinkered with. Further, outcome measurement is not only about systematic use of standardised instruments, but much work must be done after the ASI data has been collected.

    Abstract [en]

    The Swedish and the Finnish compulsory care legislation that regards abusers differ in several ways. The arguments for compulsory care and for the time which one will receive compulsory care are dissimilar. The aim of this thesis is to describe and analyse which arguments for compulsory care and the length of the care time that has been expressed in the Swedish and the Finnish laws and in the statutes. The tools were an argumentation analysis, which is a form of qualitative text analysis method, combined with a hermeneutic scientific position. We have investigated the arguments that try to justify compulsory care and the care time that has been expressed in the laws and the statutes. The next step has been to analyse the text on the basis of an argumentation analysis and the theories. The theoretical starting point has been Michel Foucault's concept of power and discipline, the paternalistic and utilitarian values and four ethical perspectives. Important conclusions are that Sweden’s arguments for applying compulsory care are to inhibit, in particular the young abusers, from destroying their lives. The care time, on maximum 6 months, is justified with the arguments that motivation for continued voluntary treatment will be provoked. The arguments in Finland has focused on removing the abuser from the society during an acute stage of the abuse, where the abuser’s life or somebody else’s life in the abusers environment is in danger, this can take place for maximum 30 days. Our results have pointed out that Sweden has a more paternalistic view to the compulsory care of abusers than Finland.

    Abstract [en]

    This study examines how gender is discursively constructed in decisions by the administrative court in Stockholm regarding the care of young persons act. Previous studies have shown a great division in how girls and boys in child welfare have been treated in Sweden. The aim of this study is to investigate whether gender is still a factor in judgments from the administrative court, examine how gender is constructed and to what consequences this can lead. The empirical material consists of six judgments from the administrative court. To analyze how gender is constructed a discourse analytic tool is used. One result is, contrary to previous studies, that boys’ sexual behaviour can also be considered a problem. The focus for boys’ sexual behaviour is, however, their actions towards others, while girls’ sexual behaviour is considered a danger towards themselves. The girls are also described in more detail and their anti-social behaviour is constructed as more severe, even though the boys are described to have more far-reaching problems. Boys do not seem to be cared for in time, which is contradictory to the care of young persons act.

    Child welfare clients represent a high-risk group for delinquency and adult criminality, but also for future suicidal behavior. We examine associations between delinquency and suicidal behavior in a national child welfare population. This register-based cohort study is based on data for all Swedish former child welfare clients born between 1972 and 1981 that experienced interventions before their adolescent years. We followed 27,228 individuals from age 20 years until 31 December 2006. Juvenile delinquency was defined as being convicted of at least one crime between age 15 and 19. The risk of suicidal behavior was calculated as incidence rate ratios (IRRs). Fifteen percent of the women and 40 % of the men had at least one conviction between the age 15 and 19. The adjusted risk of suicidal behavior among women with five or more convictions was 3.5 (95 % CI 2.0-6.2); corresponding IRR for men was 3.9 (95 % CI 3.1-4.9). Child welfare experience-specifically of out-of-home care-in combination with delinquency is a potent risk factor for suicidal behavior among young adults. However, we cannot exclude that some of this association is an epiphenomenon of uncontrolled confounders, such as impulsivity or severity of psychiatric disease. Despite this caveat, results should be disseminated to practitioners in the health and correction services.

    Objective: The influence of psychopathology on suicide method has revealed different distributions among different psychiatric disorders. However, evidence is still scarce. We hypothesized that having a diagnosis of personality disorder (PD) affect the suicide method, and that different PD clusters would influence the suicide method in different ways. In addition, we hypothesized that the presence of psychiatric and somatic co-morbidity also affects the suicide method. Method: We examined 25,217 individuals aged 15-64 who had been hospitalized in Sweden with a main diagnosis of PD the years 1987-2013 (N = 25,217). The patients were followed from the date of first discharge until death or until the end of the follow-up period, i.e. December 31, 2013, for a total of 323,508.8 person-years, with a mean follow up time of 11.7 years. The SMR, i.e. the ratio between the observed number of suicides and the expected number of suicides, was used as a measure of risk. Results: Overall PD, different PD-clusters, and comorbidity influenced the suicide method. Hanging evidenced highest SMR in female PD patients (SMR 34.2 (95% CI: 29.3-39.8)), as compared to non-PD patients and jumping among male PD patients (SMR 24.8 (95% CI: 18.3-33.6)), as compared to non PD-patients. Furthermore, the elevated suicide risk was related to both psychiatric and somatic comorbidity. Conclusion: The increased suicide risk was unevenly distributed with respect to suicide method and type of PD. However, these differences were only moderate and greatly overshadowed by the overall excess suicide risk in having PD. Any attempt from society to decrease the suicide rate in persons with PD must take these characteristics into account.

    Background As the suicide rates in young adults do not show a clear decline, it is important to elucidate possible risk factors. Juvenile delinquency has been pointed out as a possible risk behaviour. Methods This register-based cohort study comprises the birth cohorts between 1972 and 1981 in Sweden. We followed 992 881 individuals from the age of 20 years until 31 December 2006, generating 10 210 566 person-years and 1482 suicides. Juvenile delinquency was defined as being convicted of a crime between the ages of 15 and 19 years. Estimates of risk of suicide were calculated as incidence rate ratio (IRR) with 95% confidence intervals (CIs) using Poisson regression analysis with adjustment for potential confounding by their own and their parents' mental illness or substance abuse, parental education, single parenthood, social assistance, adoption and foster care. Results Among females, 5.9%, and among males, 17.9%, had at least one conviction between the ages 15 and 19 years. In the fully adjusted model, females with one conviction had a suicide risk of 1.7 times higher (95% CI 1.2-2.4), the corresponding IRR for men was 2.0 (95% CI 1.7-2.4) and 5.7 (95% CI 2.5-13.1) and 6.6 (95% CI 5.2-8.3), for women and men with five or more convictions. The effect of severe delinquency on suicide was independent of parental educational level. Conclusions This study supports the hypothesis that individuals with delinquent behaviour in late adolescence have an increased risk of suicide as young adults. Regardless of causality issues, repeated juvenile offenders should be regarded by professionals in health, social and correctional services who come into contact with this group as a high-risk group for suicide.

    Research has shown that childhood stress increases the risk of poor mental health later in life. We examined the effect of childhood stressors on psychological distress and self-reported depression in young adulthood. Data were obtained from the Child Development Supplement (CDS) to the national Panel Study of Income Dynamics (PSID), a survey of US families that incorporates data from parents and their children. In 2005 and 2007, the Panel Study of Income Dynamics was supplemented with two waves of Transition into Adulthood (TA) data drawn from a national sample of young adults, 18-23 years old. This study included data from participants in the CDS and the TA (n = 2128), children aged 4-13 at baseline. Data on current psychological distress was used as an outcome variable in logistic regressions, calculated as odds ratios (OR) with 95% confidence intervals (CI). Latent Class Analyses were used to identify clusters based on the different childhood stressors. Associations were observed between cumulative exposure to childhood stressors and both psychological distress and self-reported depression. Individuals being exposed to three or more stressors had the highest risk (crude OR for psychological distress: 2.49 (95% Cl: 1.16-5.33), crude OR for self-reported depression: 2.07 (95% CI: 1.15-3.71). However, a large part was explained by adolescent depressive symptoms. Findings support the long-term negative impact of cumulative exposure to childhood stress on psychological distress. The important role of adolescent depression in this association also needs to be taken into consideration in future studies.

    Background

    Childhood adversity (CA) is a risk indicator for psychiatric morbidity. Although CA has been linked to violent offending, limited research has considered adolescent psychiatric disorder as a mediating factor. The current study examined whether adolescent psychiatric disorder mediates the association between CA and violent offending.

    Methods

    We used a cohort of 476 103 individuals born in 1984–1988 in Sweden. Register-based CAs included parental death, substance abuse and psychiatric disorder, parental criminal offending, parental separation, public assistance, child welfare intervention and residential instability. Adolescent psychiatric disorder was defined as being treated with a psychiatric diagnosis prior to age 20. Estimates of risk of violent offending after age 20 were calculated as incidence rate ratios (IRRs) with 95% confidence intervals (CIs). Mediation was tested with the bootstrap method.

    Results

    Exposure to CA was positively associated with violent offending, especially when accumulated. Individuals exposed to 4+ CAs who were also treated for psychiatric disorder had a 12-fold elevated risk for violent offending (adjusted IRR 12.2, 95% CI 10.6–14.0). Corresponding IRR among 4+ CA youth with no psychiatric disorder was 5.1 (95% CI 4.5–5.6). Psychiatric disorder mediated the association between CA and violent offending.

    Conclusion

    CA is associated with elevated risk for violent offending in early adulthood, and the association is partly mediated by adolescent psychiatric disorder. Individuals exposed to cumulative CA who also develop adolescent psychopathology should be regarded as a high-risk group for violent offending, by professionals in social and health services that come into contact with this group.

    Childhood adversity (CA) is associated with increased risks of psychiatric disorder in young adulthood, but details in this association are less known. We aimed to explore the association of a range of CA indicators with psychiatric disorder in young adulthood, and the impact of age at exposure, disorder type and accumulation of indicators. We capitalized on Sweden's extensive and high-quality registers and analyzed a cohort of all Swedes (N = 107,704) born in Stockholm County 1987-1991. Adversities included familial death, parental substance misuse and psychiatric disorder, parental criminality, parental separation, public assistance recipiency and residential instability. Age at exposure was categorized as: 0-6.9 years (infancy and early childhood), 7-11.9 years (middle childhood), and 12-14 years (early adolescence). Psychiatric disorders after age 15 were defined from ICD codes through registers. Risks were calculated as Hazard Ratios (HR) with 95% confidence intervals (CI). Results showed that exposure to at least one CA was associated with an increased risk of psychiatric disorder (HR 1.4, 95% CI: 1.3-1.4). Risks were increased for mood, anxiety, and psychotic disorders and ADHD but not for eating disorders. The risk varied with type of disorder but was similar for all exposure periods. Individuals with multiple (3+) CAs had a two-fold risk of psychiatric disorder (HR 2.0, 95% CI: 1.9-2.1). In conclusion, our findings support the long-term negative impact of CA on mental health, regardless of developmental period of exposure. Given that experience of CA is common, efforts should be put to alleviate the burden of childhood adversities for children, particularly among the most disadvantaged.

    Background Exposure to childhood household dysfunction increases the risk of psychiatric morbidity. Although school performance also has been linked with psychiatric morbidity, limited research has considered school performance as a mediating factor. To address this gap in the literature, the current register study examined whether school performance mediates the association between childhood household dysfunction (experienced between birth and age 14 years) and psychiatric care utilisation in young adulthood.

    Methods We used a Swedish cohort of 96 399 individuals born during 1987–1991. Indicators of childhood household dysfunction were familial death, parental substance abuse and psychiatric morbidity, parental somatic disease, parental criminality, parental separation/single-parent household, public assistance recipiency and residential instability. Final school grades from the 9th year of compulsory school were used to create five categories. Estimates of risk of psychiatric care utilisation (measured as inpatient, outpatient and primary care) after the age of 18 years were calculated as HRs with 95% CIs. Mediation was tested with the bootstrap approach.

    Results Cumulative exposure to childhood household dysfunction was positively associated with psychiatric care utilisation. Specifically, individuals exposed to three or more indicators with incomplete school grades had the highest risk (HR=3.7 (95% CI 3.3 to 4.1) after adjusting for demographics), compared to individuals exposed to no indicators with highest grades. School performance was found to mediate the relationship.

    Conclusions Our findings suggest that future efforts to prevent or mitigate the negative effects of childhood household dysfunction on psychiatric morbidity may benefit from integration of strategies that improve school performance among vulnerable youth.

    Purpose

    Stressful childhood experiences have negative long-term health consequences. The present study examines the association between adverse childhood experiences, socioeconomic position, and risk of psychotropic medication in young adulthood.

    Methods

    This register-based cohort study comprises the birth cohorts between 1985 and 1988 in Sweden. We followed 362 663 individuals for use of psychotropic medication from January 2006 until December 2008. Adverse childhood experiences were severe criminality among parents, parental alcohol or drug abuse, social assistance recipiency, parental separation or single household, child welfare intervention before the age of 12, mentally ill or suicidal parents, familial death, and number of changes in place of residency. Estimates of risk of psychotropic medication were calculated as odds ratio (OR) with 95% confidence intervals (CIs) using logistic regression analysis.

    Results

    Adverse childhood experiences were associated with increased risks of psychotropic medication. The OR for more than three adverse childhood experiences and risk of psychotropic medication was for women 2.4 (95% CI 2.3–2.5) and for men 3.1 (95% CI 2.9–3.2). The risk of psychotropic medication increased with a higher rate of adverse childhood experiences, a relationship similar in all socioeconomic groups.

    Conclusions

    Accumulation of adverse childhood experiences increases the risk of psychotropic medication in young adults. Parental educational level is of less importance when adjusting for adverse childhood experiences. The higher risk for future mental health problems among children from lower socioeconomic groups, compared to peers from more advantaged backgrounds, seems to be linked to a higher rate of exposure to adverse childhood experiences.

    Background: Few studies have examined the association between adverse childhood experiences (ACEs) and disability pension (DP). The current study aimed to investigate the relationship between different ACEs, cumulative ACEs, and DP, and the mediating role of school performance. Methods: We used a Swedish cohort of 522 880 individuals born between 1973 and 1978. ACEs included parental death, parental substance abuse and psychiatric disorder, substantial parental criminality, household public assistance, parental DP and child welfare intervention. Estimates of risk of DP in 2008 were calculated as odds ratios (OR) with 95% confidence intervals (CIs). Results: A total of 2.3% (3.0% females, 1.7% males) received DP in 2008. All studied ACEs increased the odds for DP, particularly child welfare intervention and household public assistance. Cumulative ACEs increased the odds of DP in a graded manner. Females exposed to 4+ ACEs had a 4-fold odds (OR: 4.0, 95% CI 3.5-4.5) and males a 7-fold odds (OR: 7.1, 95% CI: 6.2-8.1). School performance mediated the ACEs-DP association. Conclusion: This study provides evidence that ACEs is associated with increased odds of DP, particularly when accumulated. The effects of ACEs should be taken into account when considering the determinants of DP, and when identifying high-risk populations.

    Objective: Although the relationship between childhood adversity (CA) and depression is widely accepted, there is little information on what proportion of depression is attributable to CA. Method: We used a Swedish cohort of 478,141 individuals born in 1984-1988 in Sweden. Register-based CA indicators included parental death, parental substance abuse and psychiatric morbidity, parental criminality, parental separation, public assistance recipiency, child welfare intervention, and residential instability. Estimates of risk of depression, measured as retrieval of prescribed antidepressants and/or psychiatric care with a clinical diagnosis of depression, between 2006 and 2012 were calculated as Hazard Ratios (HR) with 95% confidence intervals (CI), using a Cox regression analysis. Results: All CAs predicted depression in early adulthood. Furthermore, the predictive association between the CA indicators and depression was graded, with highest HRs observed for 4+ CAs (HR: 3.05 (95% CI 2.83-3.29)) for a clinical diagnosis for depression and HR: 1.32 (95% CI 1.25-1.41) for antidepressant medication after adjustments were made for important confounding factors. Of the studied CAs, child welfare intervention entailed highest HR for depression. Conclusion: Regardless of causality issues, children and youth with a history of multiple CA should be regarded as a high-risk group for depression by professionals in social, and health service's that come into contact with this group.

    Abstract [en]

    The aim of this study was to develop knowledge about the social workers' importance for unaccompanied children's integration in the Swedish society. In order to investigate this we also have to distinguish other people who are important for the unaccompanied children. The method used in the study is qualitative interviews and we have interviewed four unaccompanied children and three social workers to reach the results. To analyze the results we have used symbolic interactionism and Antonovsky's theory about sence of coherence. The results showed that it is important for unaccompanied children to have access to significant people to integrate and they also have to learn values and attitudes which exist in the society. It is necessary that unaccompanied children feel security in their life and that they find a trustful relationship with the significant people they meet, otherwise they won't be able to learn all those values and attitudes. We have also found that the relationship between the social worker and the unaccompanied children is uneven. The unaccompanied child perceive the social worker as a significant person in their life, while the social worker has to relate to their role as a public authority.

    Denna C-uppsats är en kvalitativ studie om hur några kontaktfamiljer ser på sina uppdrag, hur de upplever relationerna till kontaktbarnet och dess familj samt om dessa kontaktfamiljer upplever att de fått stöd eller önskar få stöd från Socialtjänsten. Undersökningen har genomförts i form av halvstrukturerade intervjuer med åtta kontaktfamiljer från tre kommuner i Stockholms län.

    Undersökningens resultat visar att det bland annat var viljan att göra en insats för ett barn och att göra något som är bra, som gjorde att dessa familjer valt att bli kontaktfamilj. Två gemensamma faktorer kan utläsas i resultatet angående vilket kontaktfamiljerna ansåg var syftet med deras uppdrag. Den ena är att kontaktfamiljerna på något sätt ville stimulera kontaktbarnet socialt och det andra är att de ville bidra till att utöka barnets nätverk. Flertalet kontaktfamiljer beskrev relationen både till kontaktbarnet och till dess familj, som god. De flesta kontaktfamiljer beskrev att de hade en sporadisk kontakt med Socialtjänsten och att kontakten skedde främst vid den halvårsvisa kontraktförnyelsen. Resultaten har analyserats och diskuterats utifrån ett systemteoretiskt och ett utvecklingsekologiskt perspektiv.

    Uppsatsen heter ”Som nyutexaminerad är man väldigt…man är lite skör och så” och är skriven av Abdiwahab Jirde och Hans Björndahl. Syftet med uppsatsen var att undersöka hur nyutexaminerade socionomer upplever utbildningens relevans för sitt nuvarande arbete samt hur de fann sig tillrätta på sin första arbetsplats. Frågeställningarna behandlade även skillnaden mellan att ha praktiserat på samma ställe eller i en liknande verksamhet. I uppsatsen användes en kvalitativ intervjumetod. Vi intervjuade åtta nyutexaminerade socionomer. Studien utgick från rollteori och socialisationsteori.

    Resultatet visade att det fanns en diskrepans mellan det som socialhögskolan lär ut och vad arbetet kräver av teoretiska kunskaper. Vissa delar av utbildningen sågs som irrelevanta medan andra delar var betydligt mer i fas med arbetsuppgifterna. Positiva moment i utbildningen sades juridiken vara vilket de allra flesta hade en omedelbar nytta av i sitt arbete. Även utredningsmomentet ansågs ha praktiskt värde. Däremot de första två terminerna i utbildningen ansågs vara mindre relevanta för nuvarande arbetsuppgifter. De teoretiska kunskaperna som förmedlades då ansågs inte ha mycket med socialt arbete att göra. Vidare menade de intervjuade att vissa praktiska kunskaper saknades och då speciellt när det gäller möten med klienter, som är påverkade av alkohol eller narkotika eller i någon form av psykiskt kristillstånd. Trots kritiken av vissa delar av utbildningen, var alla nöjda med sitt arbete och ansåg att utbildningen i sin helhet var relevant för yrket.

    This study uses qualitative research consisting of six semi-structural interviews of treatment workers who, in various capacities, have worked with men who use violence against their female partners. The purpose has been to show what the treatment counselor understand to be the reasons behind the men’s violence, what kind of backgrounds and problems they believe these abusers have and, lastly, how they view the male abusers as clients. The empirical data was analyzed through four perspectives and explanatory models derived from the past thirty years of Scandinavian research on the reasons behind men’s violence against women. Connell’s theory of masculinity was used as a filter along with the four theoretical perspectives when interpreting the data. The study revealed that the respondents found no consistent group of men who abuse. The respondents brought up reasons such as possible earlier exposure to violence, fear of abandonment, jealousy, feelings of powerlessness, and failed expectations of gender roles to be some consistent causes behind the violence. In general, according to the treatment workers, the male abusers they encounter tend to struggle with their self-image and how they fulfill perceived male ideals.

    The aim of this study was to examine how individuals who have left a criminal lifestyle may experience their present life. A symbolic interaction approach was the theoretical framework for this research. Qualitative interviews were conducted with seven individuals who have stated having had and left a criminal lifestyle. The results showed that how individuals experience their present life is highly dependent on individuals’ own view of themselves as well as on how others view them. A sense of being judged on the basis of previous lifestyle and a strive to leave the criminal identity could be discerned. There seems to be of importance for individuals to separate themselves from their prior lifestyle, both in thought and action. By doing this, they demonstrate their change both to themselves and their surroundings. By con-demning their previous destructive actions but not themselves as persons, they can maintain a positive self-image. Making good and giving back to society also tends to be of importance. Furthermore the study indicates that individuals, in addition to a sense of belonging, need to experience both resemblance and contrast in interaction in order to fully be able to leave the criminal identity. Some implications for social work practise were discussed.

    Abstract [en]

    The aim of this study is to, in view of theories about deviance and social control, examine how cannabis users are experiencing and relating to their own use and the society’s perception on this use. Qualitative interviews with six users or former users of cannabis were performed.  Sociological concepts of deviance and normality, social control, roles, role distance and stigma were used to analyze the results. The study shows that the cannabis users experience deficiencies in the Swedish drug policy. They feel like the picture of cannabis as a dangerous substance is unanimous in society and that it is not accepted to question it. They also believe that issues related to narcotics are being handled inadequately in Sweden and that this may lead to people suffering from addiction not seeking help. The cannabis users in this study also felt like other people perceived them as addicts and junkies, while the users themselves separated use and abuse. Lastly, we discovered three different strategies to deal with the attitudes and norms in society. These strategies were: resistance and questioning, hidden use and secretiveness, adapting social circle and adoption of different roles.

    This is a qualitative study about how young men in the age of 16-18 years can construct their masculinity. How do these young men describe a “real man”? What are their thoughts about themselves as adults? The purpose of this study includes a gender perspective in surroundings that cope with the sport football. To answer this study, six young men in the age of 16-18 years, whom are active football players in different levels, have been interviewed. The findings showed that the young men seemed to construct their masculinity in cooperation with the surroundings and the nearest family members seemed to have a very strong influence of the boy’s development to become a man. The results even indicated that the young men construct their masculinity by distancing themselves from what seems not to be masculine, for example male homosexuals.

    Uppsatsen är en kvalitativ studie med utgångspunkt i intervjuer med fem socialarbetare som på olika sätt i sitt arbete kommer i kontakt med flickor/kvinnor utsatta för hedersrelaterat våld. Syftet med denna studie var att beskriva hur man på socialkontor i Stockholm arbetar med flickor/kvinnor från patriarkala familjer utsatta för hedersrelaterat våld. De forskningsfrågor som har legat till grund för studien var: Hur definierar socialarbetarna hedersrelaterat våld som är riktat mot flickor/kvinnor från patriarkala familjer? Hur arbetar socialsekreterare med flickor/kvinnor utsatta för hedersrelaterat våld som vänder sig till socialtjänsten? Vad upplever socialarbetarna som hinder och vad som behöver förändras i arbetet med flickorna/kvinnorna? Har arbetssätten på socialtjänsten i arbetet med flickor från patriarkala familjer förändrats sedan 2003? Resultaten analyserades utifrån den tidigare presenterade forskningen samt teorier som ekologiska systemteorin, empowerment och några tankar om organisationens betydelse. Resultaten visade att arbetet med flickor/kvinnor utsatta för hedersrelaterat våld har förändrats till flickans/kvinnas bästa sedan 2003. Alla respondenterna är nog överens om att det fortfarande behövs fler och olika slags insatser för att ge flickorna det stöd och hjälp som de behöver.

    The purpose of this paper is to provide an overview of how cooperation functions between the health services and social services within Sollentuna County Municipal area regarding clients and simultaneous or double diagnoses. Focusing more specifically; the paper also aims to provide additionally, an inquiry as to how the implementation of the Gemensam Individuell Vårdplan (GIVP) (The Integrated Individual Service Care Programme or (GIVP) functions within the Sollentuna Municipal County Area. The questions asked in the paper are: How do the Municipal and County Council personnel experience how the (GIVP) system functions in Sollentuna? How do the Municipal Council and County Council personnel respectively describe their own and their cooperative sister services rolls in terms of the implementation of the GIVP system in practice? The projection activates a hypothesis and tries to explain how the cooperation functions and provides an assessment as to how it functions or not as may be the case in practise. The authors attempt to achieve this using a combination of methodologies. Partly through data based collection and collation through the use of a vignette-focus-study and partly through the use of a questionnaire. A combination of methods has been used to collate both quantitative and qualitative data. The analysis and interpretation of the data is managed through the use of Bengt Berggren’s (1982) cooperative model, incorporating both cognitive theory as well as meta-cognitive theory. The purpose and application of the theories is designed both to individually and collectively nuance interpretations of the relevant data. The investigations primary results show the existence of a strong willingness to cooperate. It shows that cooperation is widely regarded as being important, and that in addition, GIVP is regarded as a useful system for an improved integrated service. It additionally suggests that cooperation can be improved amongst the relevant services to an even greater extent.

    Syftet med den här uppsatsen var att undersöka hur socialsekreterare talar om att de hanterar delgivning av information till barn om att de är föremål för en utredning hos socialtjänsten. Frågeställningarna gällde huruvida socialsekreterarna säger att de delger barnen information samt hur bristande information påverkar samtalet med barnet, och även hur socialsekreterarna resonerar kring lagstiftning och etik i dessa frågor. Teoretiska utgångspunkter var Harold Öv-reeides kommunikationsteoretiska perspektiv, etisk teori och lagstiftningen. Metoden var kva-litativ och djupintervjuer i form av två fokusgrupper genomfördes. Analysen gjordes med hjälp av meningskoncentrering där centrala teman i den utskrivna intervjutexten eftersöktes. Huvudsakliga resultat var att socialsekreterarna sade sig informera barnet om utredningen utifrån vilken information det enskilda barnet kan ta till sig enligt socialsekreterarens bedöm-ning, och utifrån vad man kommer överens om med föräldrarna. Socialsekreterarna ansåg sig vara sämre på att informera barnen om utredningens avslutning och vad man i slutändan kom fram till. Det var de etiska skälen som övervägde när de informerar barnen. Socialsekreterarna tyckte inte att lagen reglerar delgivning av information till barn under 15 år.

    The opinion of social workers towards their work with children and young persons was

    studied in this essay. Even their point of view with reference to the term the child’s best

    interest has being considered and special focus was laid on the social workers attitudes

    towards media coverage and its criticism towards their work. A qualitative approach was used

    consisted of six interviews. The theoretical frameworks were based on the theory about

    freedom of action and the theory of social representations. Our result showed that almost all

    of the interviewees felt a relatively high freedom of action in their work within the framework

    of the law and other policies and that despite the difficulties this can imply, they still enjoy

    working with this target group. Concerning the term the child’s best interest, all of

    interviewees said that it is very good that it exists but that it is also difficult to define since it

    depends on whom one asks. Nearly all of the social workers thought that media coverage is

    one-sided and very negative.
